Maybe you’re slowing down a little.  You’ve been diagnosed with diabetes, high blood pressure or you’ve become a little forgetful.  Your doctor may be recommending that you look at nursing homes or think about moving in with family.  But you’re not ready.  You want to remain living at home.  In your own home.   PACE is here to help.

We’re the statewide non-profit health plan for adults 55 and older who have complex or chronic health needs and wish to remain living at home.  Founded in 2005, PACE-RI provides both insurance and medical care, as well as a range of health and social services that include adult day care, transportation, and meals at centers in East Providence, Woonsocket, Westerly and Newport. We take a comprehensive approach to well-being, and keep our participants living independently with supports. 

Participants who qualify can attend an adult day program where they will have the chance to eat, socialize, and participate in a wide range of activities. You may like music, art or nature.  Being around other people helps bring a smile to many people’s lives but we also have designated spaces within the day center if you need some quiet time.  We’ll pick you up and give you a ride, along with two delicious meals, while you’re here with us.  

Our participants come from nearly every RI community and have access to medical professionals seven days a week, 24 hours a day.  Each participant’s care has oversight by a team of providers who specialize in gerontology and regularly collaborate on ways to improve your quality of life.   

• Access to Specialists: You will have access to podiatrists, cardiologists, endocrinologists, and many other medical specialties, as a participant of PACE 

• Rehabilitation Services: Access to either center-based or in-home Physical Therapy (PT) and/or Occupational Therapy (OT)

• Transportation: Van or ambulance transport available to medical appointments

• Prescription coverage: Full prescription coverage and home delivery 

• Laboratory and x-rays: Included for PACE participants, as needed

• Prescriptions and supplies: Available at no charge and delivered to your home

We’ll also be there for you to help when you’re at home.  We have a toll-free number that you can call to speak with a medical professional at night and on weekends.  For those who qualify, we’ll provide light cleaning, shopping and assistance with household chores.  That could include grocery shopping and meal preparation for those who need it.  

Life can be complicated as you age, but becoming a PACE participant can make your life less stressful.  With our comprehensive care and coordination, we do the phone calls, scheduling and transportation arrangements for you so that you have time to live your life fully and with peace of mind.
