The PACE Way to Care is unique in today’s busy health care world: daily, we pull together an interdisciplinary team of experts to discuss the needs of our participants. Every day, those who personally take care of our participants and families in their homes, centers, and clinic rooms, come together to resolve problems and continually address the needs of our participants.

We offer a myriad of services at PACE that ensures the needs of our participants
are fully met in a plan that is specific to each and every person!
All services must be approved by our Interdisciplinary Team.
If a participant chooses to utilize non-approved services,
they may be responsible for covering those costs.
The Interdisciplinary Team
The cornerstone of PACE is the Interdisciplinary Team. Our diverse team of experts meet daily to discuss the needs of our participants. This model of care provides open communication that facilitates the quick resolution of any problems and allows for a plan for changes that will best address the needs of each individual.
