PACE Organization of Rhode Island has full-time opportunities for experienced, compassionate Home Care/Certified Nursing Assistants to provide home care services for participants in Providence, Woonsocket and South County locations.
Under the supervision of the Chief Medical Officer the Nurse Practitioner provides primary health care to PACE-RI participants including: assessments and the development of care plans in collaboration with the interdisciplinary team, provision of direct patient care and the evaluation of the effectiveness of care plans. The Nurse Practitioner functions in a collegial relationship with physicians and other health professionals making independent decisions about nursing needs and interdependent decisions with physicians regarding health regimes. Assumes dependent responsibilities in carrying out delegated medical acts.
This position reports to the Transportation Coordinator and is responsible for pick-up and delivery of passengers on a door-to door basis to and from their destination according to a pre-arranged schedule.
Reporting to the VP of Enrollment, the Enrollment Specialist is responsible for coordinating the enrollment process of potential participants into the PACE-RI program. The Enrollment Specialist actively seeks opportunities to conduct outreach activities to generate referrals. The ultimate goal of the position is to drive potential participants and/or caregivers to PACE-RI. Travel throughout the state on a regular basis is required. Enrollment meetings are typically home based.
CNA, Westerly Day Center
Under the supervision of the Day Center Director the CNA provides personal care and services according to individual care plans for participants of the PACE Organization of Rhode Island.
