Home Care Manager

Main responsibilities are to maintain and develop the home care department.  Position is responsible for the coordination and scheduling homecare services provided in the home, including home health aides, homemakers, and supplies and equipment (via the Supplies and Equipment Specialist).  Liaison for outside homecare vendors, health care professionals and internal case management staff to coordinate and oversee home care services, and any supportive services that may arise as related to participant’s individual non-clinical home care needs. Dietitian

Under the supervision of the Wellness Manager, the Dietician screens and assesses participant nutritional status. Uses pertinent data to plan and implement appropriate nutrition interventions and communicates the same to the inter­disciplinary team to ensure the nutritional needs of participants are met, including those participants who are considered to be high risk. Provides direction to staff involved in providing food service to participants, oversees dining at the PACE center and monitors staff to ensure appropriate diet orders are being followed.

The primary responsibility of this position is to provide day center participants with quality food in a safe, sanitary environment.
